We are so glad that you have decided to bring your child to our practice. Attached are forms that you can print off, fill out and bring to your appointment. It will make your check-in much faster. Please bring a copy of your insurance card and your child's immunization record to your visit. If you would like to pre-register on line please go to the patient portal and you can pre-register, you will still need to fill out the new patient forms. Please review our web site to get to know us before your appointment. Make sure and read through the immunization agreement on the new patient forms. We strongly believe that immunizations are an important part of your child's care. If you have any questions about immunizations and your child please ask the provider when you come in for your visit.
